No, do not use heat during acute pain. It may increase discomfort. Mild soreness during first use is normal and usually resolves within 24 hours. Light stretching may help.

Why does the electrotherapy function not work or feel uncomfortable?

Make sure your lower back is bare. If your skin is dry, apply a small amount of lotion to the area before use.

After using the device, my lower back hurts more. Is this normal?

Mild muscle soreness is normal. However, do not use heat therapy during acute periods of pain. Please refer to the “Troubleshooting” section in the manual to learn how to adjust usage.
